All Caravan Travelers

Travel Release Form 10079

• Everyone in the caravan must fill one out--even minors.

• If a member of your party is not here, please take one or download.

• The caravan leader must possess them before departure.

All Boat Passengers

Gary's Dive Boat Release Form

• If you are going on the boat trip you must be fill this out.

• Divers must verify their certification with the Dive Shop before the trip.

• Gary will not allow anyone on the boat without this form.

All Certified Divers

Certified Divers Form 10086

• All certified divers must complete PADI form 10086 before diving.

• Fill it out and hand it to the trip leader.

Single Parent/Guardian

Single Parent Permission Form

• All parents traveling singularly with a minor child abroad, must have the appropriate permission to travel to a foreign country.

• There is also PADI form 10088 which is a medical release form you may wish to review.

Continued Ed Students

Continued Ed Admin Form 10038

• Everyone enrolled in a course beyond Open Water certification is required to complete this form.

• This does include current OW students taking a concurrent specialty certification.

• All students must submit a medical questionnaire within the last 12 months.

Special Student Forms

• Under certain circumstances a student may be required to submit other forms. Please check with your instructor for all proper document requirements before your departure.
